You have one set of adult teeth to last your lifetime. Make it easier to take care of them. As a dental health practice we have a preventive approach to dentistry. Working together we want you to have: • Fresher breath and more attractive teeth • Prevention of tooth decay and gum disease • Fewer fillings and reduced tooth loss • Less risk of toothaches or abscesses • Screening and prevention of oral cancer • Your own teeth for life • Fewer and less complex visits • Less cost What does Membership include? • Regular dental health reviews at six, nine or twelve month intervals dependent on your needs, providing: • detailed dental health assessments • screening for oral cancer and dental disease • advice on any dental health, functional or cosmetic aspect. • Regular hygienist appointments at three, six, nine or twelve month intervals dependent on your needs, providing: • prevention of bad breath, cleaning and polishing • preventive knowledge and techniques to reduce tooth decay and gum disease • stain removal and maintenance of a clean fresh smile • Radiographs – all X-rays are included. • 20% reduction in fees for new patient assessments and initial hygienist visits to recover health. • 10% reduction in fees for ANY additional treatment (membership loyalty bonus), including: Do I need to be Dentally Fit to Join? No. We recommend that every patient join the Practice Membership Plan as soon as possible. Unlike other dental plans the state of your mouth is not assessed for past disease or any outstanding treatment beforehand. Choose the level of membership that suits your needs: There are four membership levels to match how regularly you require Dental Health Reviews and Hygienist appointments. For example if you see a hygienist every 3 months, Membership 3 is the most suitable level for you. We can recommend the level most appropriate to maintain your dental health. Membership 12 £7.20 per month (£1.66 per week) Membership 9 £8.50 per month (£1.96 per week) Membership 6 £11.15 per month (£2.57 per week) Membership 3 £16.15 per month (£3.73 per week) Payment is by direct debit. Please note that with your first monthly payment there is a once only set-up fee of £10. How do I Join? Simply ask for a form at reception and take a few minutes to fill it in. You will need to sign it and complete your bank account details on the Direct Debit mandate.
